  Dear Patient,

patient surveyWe would like to know how we can improve our service to you and how you perceive our surgery and staff. 

Our Patient Participation Group (PPG), formed in September 2011,  is a group of volunteer patients working with us to survey patients' views on our services and trying to meet the challenges that changes within the NHS are bringing. We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups. 

We are happy for patients to keep in touch with us by email - complete an application form from the link below and when we have your email address we can contact you with newsletters or future surveys.
